Your right to a Good Faith Estimate

Under the No Surprises Act, you have the right to receive a Good Faith Estimate of expected costs for your care before your first appointment. This applies if you are uninsured or choose not to use your insurance. Our team will provide a written estimate covering the cost of services expected to be rendered during your course of treatment.

  • You have the right to dispute a bill if the final cost is $400 or more above the Good Faith Estimate.
  • Ask our front desk for your written Good Faith Estimate before any scheduled services.
  • This notice does not waive your right to use insurance or affect your legal rights.
